Policies promoted and adopted for allocating ventilators during the COVID-19 pandemic have often prioritised healthcare workers or other essential workers. While the need for such policies has so far been largely averted, renewed stress on health systems from continuing surges, as well as the experience of allocating another scarce resource—vaccination—counsel revisiting the justifications for such prioritisation. Prioritising healthcare workers may have intuitive appeal, but the ethical justifications for doing so and the potential harms that could follow require careful analysis. Ethical (...) justifications commonly offered for healthcare worker prioritisation for ventilators rest on two social value criteria: instrumental value, also known as the ‘multiplier effect’, which may preserve the ability of healthcare workers to help others, and reciprocity, which rewards past usefulness or sacrifice. We argue that these justifications are insufficient to over-ride the common moral commitment to value each person’s life equally. Institutional policies prioritising healthcare workers over other patients also violate other ethical norms of the healthcare professions, including the commitment to put patients first. Furthermore, policy decisions to prioritise healthcare workers for ventilators could engender or deepen existing distrust of the clinicians, hospitals and health systems where those policies exist, even if they are never invoked. Tax compliance is a concern to governments around the world. Prior research (Alm, J. and I. Sanchez: 1995, KYKLOS 48, 3–19) has attributed unexplained inter-country differences in compliance rates to differences in social norms. Economics researchers studying tax compliance in the United States (U.S.) (see for example J. Andreoni et al.: 1998, Journal of Economic Literature 36, 818–860) have called for more attention to social (as opposed to economic) influences on tax compliance. In this study, we extend this prior research (...) by explicitly examining the role of social norms [Cialdini, R. and M. Trost: 1998, The Handbook of Social Psychology (Oxford University Press, New York)] on tax compliance in three different countries. We test our research hypotheses using a hypothetical compliance scenario, which was administered in Australia, Singapore, and the U.S. There were differences in compliance rates and social norms among the three countries. Factor analysis of the social norm questions identified three distinct social norm constructs. Two of these factors were significant in explaining tax compliance behavior. The first and most influential factor was taxpayers’ own personal moral beliefs, along with the beliefs of those close to them (e.g., friends and important others). The second significant factor represented societal views of proper behavior. We conclude that social norms help to explain tax compliance intentions and why tax compliance rates are higher than would be predicted by strictly economic models. (shrink)

Nurses have moral obligations incurred by membership in the profession to participate knowingly in health policy advocacy. Many barriers have historically hindered nurses from realizing their potential to advance health policy. The contemporary political context sets additional challenges to policy work due to polarization and conflict. Nursing education can help nurses recognize their role in advancing health through political advocacy in a manner that is consistent with disciplinary knowledge and ethical responsibilities. In this paper, the authors describe an exemplar of (...) Elizabeth Barrett's “Power as Knowing Participation in Change” theory as a disciplinary lens within a doctoral nursing health policy course. Barrett (radically) emphasizes “power as freedom” instead of “power as control.” This approach is congruent with nursing disciplinary values and enhances awareness of personal freedom and building collaborative relationships in the policy process. The theory was used in concert with other traditional policy content and frameworks from nursing and other disciplines. We discuss the role of nursing ethics viewed as professional responsibility for policy action, an overview of Barrett's theory, and the design of the course. Four student reflections on how the course influenced their thinking about policy advocacy are included. While not specific to policymaking, Barrett's theory provides a disciplinary grounding to increase students' awareness of freedom and choices in political advocacy participation. Our experience suggests that Barrett's work can be fruitful for enhancing nurses' awareness of choices to participate in change across settings. (shrink)

Surprisingly, very little has been said on the matter since, even by the most naturalistically inclined metaphysicians. Here, we revisit the Katzav–Ellis arguments of 2004–05. We outline the two problems for the dispositionalist identified Katzav in his 2004 , and claim they are not as problematic for the dispositional essentialist at it first seems – but not for the reasons espoused (...) by Ellis. Many necessitarians about cause and law (Armstrong 1983; Mumford 2004; Bird 2007) have argued that Humeans are unable to justify their inductive inferences, as Humean laws are nothing but the sum of their instances. In this paper I argue against these necessitarian claims. I show that Armstrong is committed to the explanatory value of Humean laws (in the form of universally quantified statements), and that contra Armstrong, brute regularities often do have genuine explanatory value. I finish with a Humean attempt (...) at a probabilistic justification of induction, but this fails due to its assumption that the proportionality syllogism is justified. Although this attempt fails, I nonetheless show that the Humean is at least as justified in reasoning inductively as Armstrong. (shrink)

Philosophy of Medicine has for a long time been preoccupied with analyzing the concepts of health, disease and illness. Relatively speaking, the concept of medicine itself has received very little attention. This paper is a contribution to the relatively neglected debate about the nature of medicine. Building on the work of Alex Broadbent (Broadbent, 2018a, b), Chadwin Harris (Harris, 2018) and Thaddeus Metz (Metz, 2018), in this paper I question the persuasiveness of Broadbent’s account of the “core business” of medicine, (...) The Inquiry Thesis, and propose an alternative: a revised version of The Curative Thesis, which I coin The Best Available Intervention Thesis. Crudely, this suggests that Medicine is the sustained and organised effort to determine and prescribe the best available treatment (curative or preventative) for patients. (shrink)

Slowly, we are gaining a deeper understanding of the persisting psychological trauma experienced by students at colonial universities, and beginning to recognize that the Eurocentric curricula and pedagogies must change if students such as the “born-frees” in post-Apartheid South Africa are to flourish. In this article, I present a sub-Saharan African concept of “the ethical teacher,” and use this to ground a “ubiquitous action-reaction” teaching model. I use these concepts to develop a decolonized pedagogy – a teaching methodology that avoids (...) a number of harmful colonial teaching practices in philosophy. I suggest a number of novel ways of accommodating a “decolonized education” with a view to inspiring teachers of philosophy in colonial countries globally. I propose a new, malleable pedagogical model that is particularly useful in the colonial context, since its uniqueness lies in the African ethical framework that grounds it. However, I contend that philosophy educators globally will benefit from taking the principles proposed in this article seriously. (shrink)

In this paper I untangle a recent debate in the philosophy of epidemiology, focusing in particular on the Potential Outcomes Approach to causation. As the POA strategy includes the quantification of ‘contrary-to-fact’ outcomes, it is unsurprising that it has been likened to the counterfactual analysis of causation briefly proposed by David Hume, and later developed by David Lewis. However, I contend that this has led to much confusion. Miguel Hernan and Sarah Taubman have recently argued that meaningful causal inferences cannot (...) be drawn from obesity. This paper prompted Alex Broadbent to criticise the POA conception of causation, accusing two of the four theses its proponents are committed to, of circularity and falsity. Here I scrutinise Broadbent’s claims, and suggest that a Popperian approach to causal inference in epidemiology diffuses both of his objections. However, I move on to argue that the POA’s commitment to granting only manipulable conditions causal-status, renders the position implausible as a conceptual analysis of causation. That said, I conclude that the strategy the POA employs is an effective tool for effect-measurement in intervention-cases; if it is a conceptual analysis of causation at all, it must be restricted to the causal analysis of manipulable conditions. The POA’s failure to successfully demarcate causal from non-causal conditions simpliciter should therefore not be viewed as a serious threat. (shrink)

Interdisciplinary research assumes that teams of highly specialized scientists develop new knowledge by bridging their respective horizons. Nurse educators preparing nursing doctoral students to conduct interdisciplinary research need insight into how members of interdisciplinary research teams experience knowledge horizons in these complex contexts. Based on the work of the philosopher Bernard Lonergan, this pilot study uses Transcendental Method for Research with Human Subjects to explore interdisciplinary researchers' experiences with and attitudes toward interdisciplinary research. Results reveal the overarching conceptual category of (...) “engaged interdisciplinary inquiry” which includes six themes: (i) valuing interdisciplinary engagement; (ii) direct engagement; (iii) interior engagement; (iv) disengagement; (v) facilitated engagement and (vi) engaged researcher development. Results also suggest engagement depends on vigorous “back and forth”, or dialogue, with self and others, and demonstrate the study method is fruitful for cognitive inquiry. This pilot supports expanded study to inform preparation for and conduct of interdisciplinary research involving nurses and raises important questions about how the trend toward interdisciplinary research affects nursing science. In a recent discussion about how scientific knowledge might potentially change our understanding of the nature and extent of human genetic, cultural, or biological variation, the sociologist David Skinner identified two competing visions of the future: one that was decidedly dystopian, which conjured up a “re-racialized” future, and an opposing utopian future in which the potential for racialized thinking might be finally overcome. We can situate the ongoing debates about the congestive heart failure drug BiDil, approved by the Food and (...) Drug Administration for use only by African Americans, in relation to these differing future prospects.When the FDA announced its approval of BiDil in June 2005, it located the drug, and perhaps the future of pharmaceutical development, within a particular vision of the future, heralding BiDil as “representing a step toward the promise of personalized medicine.” The discourse of “personalized medicine” can be characterized as part of a utopian future, one in which clinicians will be able to make increasingly individualized decisions based on each patient’s genetic makeup so that the drugs they take will be those that work best for them. Background:Clinical investigation is a growing field employing increasing numbers of nurses. This has created a new specialty practice defined by aspects unique to nursing in a clinical research context: the objectives, setting, and nature of the nurse–participant relationship. The clinical research nurse role may give rise to feelings of ethical conflict between aspects of protocol implementation and the duty of patient advocacy, a primary nursing responsibility. Little is known about whether research nurses experience unique ethical challenges distinct from those experienced (...) by nurses in traditional patient-care settings.Research objectives:The purpose of the study was to describe the nature of ethical challenges experienced by clinical research nurses within the context of their practice.Research design:The study utilized a qualitative descriptive design with individual interviews.Participants and research context:Participating nurses self-identified as having experienced ethical challenges during screening. The majority were Caucasian, female, and worked in outpatient settings. Approximately 50% had > 10 years of research experience.Ethical considerations:The human subjects review board approved the study. Written informed consent was obtained.Findings:Predominant themes were revealed: the inability to provide a probable good, or/do no harm, and dual obligations. The following patterns and subthemes emerged: conflicted allegiances between protocol implementation, needs of the participant, desire to advance science, and tension between the nurse–patient therapeutic relationship versus the research relationship.Discussion:Participants described ethical challenges specific to the research role. The issues are central to the nurse–participant relationship, patient advocacy, the nurse’s role in implementing protocols, and/or advancing science.Conclusion:Ethical challenges related to the specialized role of clinical research nurses were identified. I am foregoing the discussant’s critical role in favor of a short examination of how one sociologist’s imagination is tantalized and irritated by some of the ideas and interconnections of Professor Messick’s paper. The question is, when it comes to ingroups and outgroups, why does race matter? Why does sex or gender matter? I will briefly make four points about sociobiology, favoritism toward the ingroup, hostility toward the outgroup, and finally, the conflict theorist’s favorite topic — resource allocation.

Pancreatitis is a complex, progressively destructive inflammatory disorder. Alcohol was long thought to be the primary causative agent, but genetic contributions have been of interest since the discovery that rare PRSS1, CFTR and SPINK1 variants were associated with pancreatitis risk. We now report two associations at genome-wide significance identified and replicated at PRSS1-PRSS2 and X-linked CLDN2 through a two-stage genome-wide study. The PRSS1 variant likely affects disease susceptibility by altering expression of the primary trypsinogen gene. The CLDN2 risk allele is (...) associated with atypical localization of claudin-2 in pancreatic acinar cells. The homozygous CLDN2 genotype confers the greatest risk, and its alleles interact with alcohol consumption to amplify risk. We report the identification of a recurrent, 520-kb 16p12.1 microdeletion associated with childhood developmental delay. The microdeletion was detected in 20 of 11,873 cases compared with 2 of 8,540 controls and replicated in a second series of 22 of 9,254 cases compared with 6 of 6,299 controls. Most deletions were inherited, with carrier parents likely to manifest neuropsychiatric phenotypes compared to non-carrier parents. Probands were more likely to carry an additional large copy-number variant when compared to matched controls. The clinical (...) features of individuals with two mutations were distinct from and/or more severe than those of individuals carrying only the co-occurring mutation. Our data support a two-hit model in which the 16p12.1 microdeletion both predisposes to neuropsychiatric phenotypes as a single event and exacerbates neurodevelopmental phenotypes in association with other large deletions or duplications. Analysis of other microdeletions with variable expressivity indicates that this two-hit model might be more generally applicable to neuropsychiatric disease. © 2010 Nature America, Inc. I am a published author and the mother of one daughter and two sons. I have six grandchildren. I just wouldn't like doing anything else. I love my job and family and my teacup poodle, Baby!Let's say I have talked to friends who are not STNAs about my job. I tell them that there is always going to be someone's grandma, grandpa, sister, brother, aunt, uncle, mother, father, etc. who will one day need to be placed in a home. I tell them how good I feel when I can help them. STNAs are not going away, and the door to the home I work at will not close. In some cases it's like taking care of a child. The way you give them a bath, dress them, feed them, take care of incontinence problems. No, I agree, not everyone can do my job. I hear that from my friends a lot. So I tell them, I thank God I can do the job I have.What I do when the resident comes to our facility, I introduce myself to them and to their family. I ask the resident, "Are you spoiled?" Sometimes they say no not at all. I tell them, "Well you are in for a treat. I love to spoil and that's one of the things we do around here." If they say yes, I tell them, "Well we have our work cut our for us, because we have to spoil you even more." I usually get lots of smiles from the family and resident. After they settle for a while, and the family is alone, I ask the [End Page 153] family questions about their loved one. I like to get a story about the resident from family so I can use it in my conversation later with the resident. I assure the family their loved one will be treated exactly the way they themselves would treat them. We are not trying to replace the family. We are filling in as family while they are away from the loved one. Then after the family is gone, I keep checking in on the resident, visit for a spell and chat. I ask them if they need anything. I just want the resident to feel safe, loved, and comfortable.Demanding family members can mean different things. For example, if they come in to help feed their loved one—I had a family member tell me, "I want mother's food now so I can feed her." I tell the family the cart that is coming now is room trays and as soon as we deliver the room trays, because we want our residents to get hot meals, I will call the kitchen and get that tray for your mother ASAP. Some family members just want you to drop all you are doing to assist their loved one. When a family member asks for something we are pretty prompt to get up and take care of it. It assures them that we will not put off anything asked of us. Here he consciously addresses the problems raised by supposing realistically that we are dealing for the most part with more or less recognizable traditions and subtraditions embodied in various communities. Second, the logic of doctrinal claims has not been explored systematically before in relation to both their truth (or rightness, where they incorporate practical tenets) and their authenticity, as genuine do.ctrines of a given community. Third, the book raises some vital questions about the present shape of the world. For instance, the way in which the analysis fits non-religious but analogous cases (e.g. Romanian Marxism) is important; as also the question of how modern eclecticism fits into the picture. It appears to me obvious that in many respects ideologies such as Marxism fulfil a function, or set of functions, very similar to those fulfilled by religions. Black Feminism and Continental Philosophy in dialogue.

The author has collaborated in research teams to visualize supercomputer simulations and real-time data. She describes these collaborative projects that employ advanced-technology graphics and novel digital displays that include large-format IMAX film, high-definition television productions, and a museum digital dome at the American Museum of Natural History. The popularity of these images and the function that they provide in popular culture are discussed. She also describes two key technologies that she was part of designing: IntelliBadge(tm), a real-time visualization and ‘ (...) class='Hi'>smart’ tracking system; and Virtual Director(tm), a virtual camera choreography and remote collaboration system. The process of data-visualization involves the mapping of data from numerical form into an iconic representational form in the attempt to provide humans with insight and understanding of a phenomenon. This is discussed in the context of metaphor, cognition, and postcolonial theory. Because data-visualizations carry the ‘weight of scientific accuracy and advanced technology’, most general audiences confuse these visualizations as ‘real’; however, it is argued that data-visualizations are models and metaphors, not reality. In metaphor theory, the mapping of attributes from one domain of information into another is how humans understand, create, and engender new meaning. Data mapping is correlated with this theory. The author explores how the use of mapping information is culturally contingent. In the spirit of scientific inquiry, she deconstructs the very professional activity for which she is most famous. When parents must rely on public assistance and family shelters to provide for their children's most basic needs, they lose autonomy. Within a system of public assistance that already stigmatizes and isolates its beneficiaries, their family lives become subject to public scrutiny and criticism. The author uses the Massachusetts family shelter system to explore the impact of (...) asset and deficit-oriented help-giving approaches as they are experienced by mothers and service providers. The format of the book is unique. Following each chapter are the "reflections" of a mother who has parented in a shelter, a front-line worker, and a shelter director. The author and contributors propose a "Power With" policy and practice framework that runs counter to the prevailing "Power Over" cultural policy trends. Can't Regulate, Won't Regulate? As the global trade in human eggs continues to expand with logarithmic momentum, it is frequently argued that we could not regulate it even if we wanted to. Not all commentators do want to, of course. Many view regulation as counterproductive: reports have suggested that FDA governance has had the perverse effect of increasing levels of reproductive tourism to Latin America. Most of the other chapters in this volume are broadly in favour of letting market forces (...) take their course, with varying degrees of control. I will examine whether reproductive freedom and choice really are all-important. Before I begin that analysis, however, I need to clear away the argument that we cannot control black markets in human body parts, including eggs, even if we want to. This argument is common to commentators from many positions in the political spectrum. Some writers assert that “baby markets,” comprising egg sale, surrogacy, and paid adoption, are so firmly established that prohibiting or regulating them salves our collective conscience but amounts to no more than hypocritical pretense. Other commentators claim that the burgeoning demand for eggs simply makes even an honest attempt to ban markets unrealistic or imply that the natural and laudable desire for “procreative liberty” would inevitably drive couples to defy paternalistic legal bans. On any of these accounts, regulating global markets in human tissue is said to be a lost cause. I argue the opposite. (shrink)

How can intellectuals who oppose the illegitimate war in Iraq come to similar terms with the U.S. neoconservatives, and their unrepentant British collaborators, who have stranded us in it? In the Tempest, Shakespeare’s most political play, comedy though it is meant to be, intellectuals are warned not to consider themselves guiltless. But how can those who marched against the war, or who tried to speak truth to power in other ways, be guilty of its misuses? Surely this is too harsh (...) a view of the role of the public intellectual. What choice did Prospero really have? He was exiled, hapless, stranded. This chapter explores critically ethical concerns arising from forms of suffering to which domesticated nonhuman animals are subjected in scientific instruction and research and within the industrial-factory-farm-food complex, as well as other contexts. Consideration is given to the views of Arthur Schopenhauer on suffering, René Descartes’s designation of ontological differences between human and non-human animals, and Donna Haraway’s reconfiguration of the relationship between human and nonhuman animals in scientific laboratory settings. Proceeding from a discussion of David Benatar’s “antinatalist” views (...) the focus of analysis is on the forms of suffering imposed on domesticated nonhuman animals by humans. In response to ethical concerns raised about the suffering inflicted on nonhuman animals in the course of scientific research, scientists have sought a “solution” in the form of genetically engineered nonhuman animals whose responses to painful stimuli are presented as modulated to reduce pain. This reductive conceptualization of suffering reduces the complexity of suffering to physical sensation alone and does not engage with the ethical issues involved. Drawing on the work of Emmanuel Levinas and Albert Schweitzer the chapter concludes that an ethical solution to the complex issues explored lies in refraining from exposing nonhuman animals to pain and suffering.
